Please submit your application through the HKV-Award Online Application System. When registering an account, please select that "you are a corporate member of HKCV under AVS" and fill in your membership number and the phone number registered in HKCV.

Corporate members' records of volunteer services referred by HKCV (AVS Hours) were transferred to the HKV-Award Online Application System, and were pre-input to the relevant application forms.
Please input your volunteer service records (other than the AVS Hours) in the application form, if necessary.

The application form cannot be modified once submitted. Please carefully review all the filled information before submission.
